Backups and record deletion SLAs

I was wondering how people handle their record or file deletion SLAs.

For example say you work for a hospital and a patients record needs to be removed from your applications database. You do this but the patient still exists in all of your backups. If you have backups to tape it becomes even more unwieldy to remove this record but still keep everything else.

Do you just make sure every time that backup is restored moving forward that record is deleted each time or are people actually restoring there backups deleting records and creating new backups. I find it more likely that a script is updated to delete the records that were agreed to be removed each time a restore might be required in the future.

It seems like it would be a massive time investment to purge data from old backups unless I'm thinking about this the wrong way. Thought...
